What’s Included: A 3-hour one-on-one meeting with me, a licensed clinical social worker trained in adult ADHD evaluation A comprehensive biopsychosocial history, covering everything from lifelong patterns to current life stressors Administration of key validated assessment tools: DIVA 5 – Detailed diagnostic interview based on DSM-5 ADHD criteria BDEFS – Barkley Deficits in Executive Functioning Scale WFIRS‑S – Weiss Functional Impairment Rating Scale – Self‑report While the WFIRS‑S may not feel the most affirming, it’s a unique, psychometrically robust tool that uncovers real-life impact leading to much more accurate, meaningful diagnoses. Plus any additional measures needed to tailor accuracy to your unique profile 🚫 Note: This is NOT a neuropsychological evaluation. The focus here is on clear diagnostic clarity and practical understanding. You have two options after your evaluation: Option 1: Diagnosis Confirmation Letter – $375 Includes the full 3-hour evaluation and a formal letter confirming your diagnosis and summarizing clinical impressions. This is ideal for people who want validation, clarity, and documentation to: Share with their primary care doctor or psychiatrist, begin therapy or coaching with an ADHD-informed provider, use for basic workplace/school support (when a brief summary is sufficient) Option 2: Comprehensive Diagnostic Report – $495 Includes everything from Option 1 plus a full written report detailing: Your symptom profile and diagnostic criteria, results from each assessment measure, personalized interpretation and functional impact.
